Putnam NY Connects provides a plethora of services for individuals looking for Long Term Care, however the general service we provide are Information and Assistance, Needs Assessments and Public Education.

Information and Assistance
An objective and comprehensive information is gathered from each Putnam resident to determine the appropriate institutional service and or eligibility criteria to link them with the services and resources available to help meet their particular needs.

Public Education
In an effort to reach as many people as we can Putnam NY Connects will have on-going education and awareness campaigns in and around the community about long term care, including long term care insurance.

Needs Assessments
To ensure appropriate needs are being met a preliminary evaluation is done for all individuals to identify any social, medical and financial needs as well to determine the available service options and choices to meet these needs.
