Nominations for our 2013 Youth Volunteer Awards

To be eligible for consideration, all nominations must reach the
Youth Bureau by Deadline: Friday, January 25^th, 2013 @ 12 Noon

We have attached the following documents for your convenience:

a)    2013 YOUTH Nominator’s Packet -3 pgs
b)   2013 ADULT Nominatior Packet- 3 pgs

To Nominate A Volunteer:  

• READ our Letter detailing the Youth Awards Process
  
• PRINT the Nominator’s Packet for Youth or Adult (your choice)
  
• COMPLETE Nomination & return to us by Jan. 25, 2013 @ 12 noon
  
• Electronic Submission Preferred  youthawards@putnamcountyny.gov
